We are offering a FREE flu shot clinic for all faculty and staff, and their families. (NOTE: This is a different clinic than the one for CTS students.)

See recent faculty/staff email from Human Resources for list of insurance providers accepted and for various consent forms needed.

To schedule your appointment, please follow the instructions below.

1. Schedule one appointment per person.

2. Click the following link  

3. Enter your email address.

4. If you have not used PickATime before, you will receive the message “Your email address is not registered. Please click HERE to sign up.” Click HERE in the pink/red box.

5. Type your first and last name and create a password.

6. Click “Create Your Free Account.”

7. Once you are logged in, click on “91���� Staff.”

8. Select the appointment time you want.

9. Check the box that says, “I would like to add this appointment to my calendar.”

10. Select “Create Appointment.”

11. Done. You can print if you would like.

**Please bring your insurance information and consent form(s) to your scheduled appointment(s).
